Why does the root cause/corrective action (RCCA) process fail to achieve the desired results? 

The process consists of people who are carrying out various activities in order to collect information on the failure situation, identify possible causes, and collect and analyze data in order to drill down into the causal relationships to find which ones are relevant and need to be prevented.

The barriers then have to do with the process being used to initiate and guide the analysis, and the knowledge and skills of the people involved. In this presentation expert speaker Duke Okes will provide you with a list of eight specific barriers to effective RCCA often found in organizations, and provide proven solutions to address them.
