5 Top Tools for Root Cause Analysis (RCA)

Root cause analysis (RCA) is used by organizations to respond to both critical incidents (e.g., accidents, major breaches or outages) and repetitive problems (complaints, errors, audit nonconformities). The intent is to identify systemic causes that if removed, will prevent reoccurrence of the issue.  However, evidence indicates that such efforts fail to achieve the expected results.Much of this can be attributed to the widespread use of what Daniel Kahneman calls System 1 thinking, instead of a more robust analysis based on a rational search.

This presentation by expert speaker Duke Okes will demonstrate how the use of System 2 thinking, combined with the right visual tools, can significantly increase the effectiveness of problem diagnosis.
