How Root Cause Analysis Helps To Prevent Potential Future Problems

Understand a process that allows organizations to turn their failures into learning events, but before everything hits the fan! Based on the highly visible failures evident during the past few decades (e.g., in the food chain, automotive industry, and pharmaceuticals), it is obvious that many organizations need to do things differently if they don’t want their failures to be on the front page of the newspaper (or web page, twitter stream, etc.).

Too often the root cause/corrective action process in an organization is relegated to personnel who deal primarily with customer complaints, audit findings, employee injuries, and equipment failures. However, those who understand the need for both proactive and reactive risk management processes recognize it as a key component of effective governance. This presentation will uncover some of the key reasons root cause/corrective action isn’t sometimes seen as an executive responsibility and some barriers to its effective implementation as such.
